Community Sister/District Nurse - 37.5 hours per week

We are looking for dynamic and enthusiastic Community Sisters/District Nurses to join our Primary Care Network (PCN) teams in Crawley and Horsham in the first instance. There may also be future opportunities in other teams based in Burgess Hill, East Grinstead, and Haywards Heath which we’d be happy to explore with successful candidates.

The emerging PCNs are groups of practices working together in collaboration with community and other health and care services. PCNs enable greater provision of proactive, personalised, coordinated and integrated health and social care.

The Community Sisters/District Nurses have overall responsibility for a designated caseload and will manage care delivery by the integrated team. The post holders will attend health and social care meetings to discuss complex patients at risk of admission and devise contingency plans.

They will also be responsible for staff management, including personal development reviews, supervision and staff development, and expected to take the lead for the team on a rota basis for handover and allocation purposes. Applicants should be experienced Community Nurses with a wide range of clinical competencies and a mentorship qualification.
