An exciting opportunity has arisen to join our Community Rapid Response Team in Bassetlaw.

We have one full time permanent position available: 37.5 hours/ week.

We are looking for an individual who has had experience of working in a healthcare or social care setting. You will be highly motivated and work well in a team or on your own. You will need the ability to adapt to changing priorities and work in a busy environment.

Our service offers hospital at home & intensive rehabilitation services to people requiring care & physical rehabilitation to enable timely discharge from hospital and admission avoidance.

You will be involved in the assessment, treatment and rehabilitation of patients, working alongside a large Multi-disciplinary team.

The post involves working shift work, including weekends & bank holidays - the current hours of the service are 7am- 9pm. A full induction programme and training will be available for the successful applicant.

You will work both individually and along side other members of the Rapid Response Team which consists of Clinical Assessors, Physiotherapists, Occupational Therapists, Assistant Practitioners , Community Rehabilitation Assistant and Social Workers who all work together in providing nursing & rehabilitation support for adults to enable them to improve their function, maintain their independence and achieve their goals.

The duties of this post include carrying out delegated therapy treatment programme, supervising and progressing patients goals. You will also support patients with their personal care needs with an enabling approach.

Strong communication skills are essential to ensure patients, relatives and the multi-disciplinary team are kept informed verbally and via clinical notes & IT systems.

Good organisational skills are key to the smooth running of the service and the ability to work flexibly to support the needs of the service.
