An excellent opportunity has arisen for a qualified nurse to join our forward-thinking, community out-of-hours team in North Tyneside. We provide care to residents in the North East of England which is renowned for its friendly people, outstanding coastline & beautiful countryside.

You will be required to work a varied shift pattern to fulfil the clinical & management aspects of the post. Evening & nightshifts attract enhanced payments in line with NHS Terms & Conditions of Service.

The post holder may be required to work at other sites within the Trust.

You will manage, supervise & develop a team of community nurses who provide a broad range of nursing care (palliative/end of life care, injectable medication, wound & catheter care, IV therapy etc) to patients during the out of hours period.

You will undertake advanced clinical assessments & play an important role in reducing unplanned admissions & facilitating earlier discharge from hospital for patients through collaborative working with day services & MDT meetings.

You will demonstrate advanced professional practice including medicines management, leadership, supporting self-care management & forge interagency & partnership working. The post-holder will work proactively to reduce unplanned hospital admissions & facilitate earlier discharge from hospital for patients identified through multi-disciplinary team meetings
