An exciting opportunity has arisen to join the Continuing Healthcare team. We require Registered Nurse Adults to join the Continuing Healthcare Team.

The Continuing Health Care team are an innovative and friendly team who deliver a high quality service to the population of Wirral in order to assess and consider continuing healthcare eligibility.

We are looking for experienced Registered Nurses to join CWP within the CHC Team as a Individual Commissioning Nurse . Our Nurses look at the whole process, by first determining eligibility for NHS funding through utilisation of the Decision Support Tool. The work is at times challenging but also rewarding. The position available is full time permanent and will be a valuable professional development opportunity for applicants wishing to gain experience in Continuing Health Care and care management of individuals with complex ongoing care needs to inform their future practice.

Cheshire & Wirral Partnership (CWP)are looking to recruit three experienced and motivated Registered Nurses to join our CHC Team.

The CHC Team are responsible for assessing and determining eligibility for people with complex health needs whom are eligible for NHS funding for long term care and treatments under The National Framework for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care(FNC).

Whilst travel is limited during the Covid-19 pandemic and much work has been done virtually this role ordinarily involves a considerable amount of travelling across the North West therefore access to transport is essential.
