Job summary

To complement our friendly and supportive team, we are looking for an enthusiastic and caring GP to work between 4-8 sessions per week. Working patterns and times can be negotiated to suit personal circumstances. A competitive salary is offered to reflect experience.

Main duties of the job

All GPs have a share of routine consultations, duty days, home visits, workflow admin and tasks.

Patient consultations are either in-person at the surgery, or remote by telephone or video. Home visits are shared between GPs and the new Home Visiting Service.

We are also a Teaching Practice for medical students from the University of Nottingham and share the mentoring across our GP team.

Heartwood belongs to the Swadlincote PCN group of 5 surgeries and have access to shared ARRS funded roles to support us in practice.

We are an innovative practice, continuously seeking improvements in delivering high quality patient care.

Our team is friendly and supportive, with high levels of experience. Consisting of 5 GPs, 3 ANPs, Pharmacist, Practice Nurses, TNA, HCAs, Health coach, Social Prescriber, Care Co-ordinators and reception/admin team.

We are also a Teaching Practice for medical students from the University of Nottingham and share the mentoring across our GP team.

Clinical Responsibilities

  • The GP will undertake a variety of duties including Practice consultations, including telephone, video and face to face. Handling patient queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
  • Admitting or discharging patients to and from the caseload and referring to other care providers, as appropriate.
  • Recording clear and contemporaneous consultation notes to agreed standards, using the clinical computer system.
  • Participating in QOF/IIF related work.
  • Compiling and issuing computer generated acute and repeat prescriptions.
  • Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  • Perform appropriate skilled examinations with consideration for the patient.
  • Respect patients right to confidentiality and provide information to colleagues.
  • Respond rapidly to emergencies.
  • Ensure up to date emergency equipment and drugs used if required.
  • Ensure up to date with developments in clinical practice and regularly review knowledge and performance.
  • Use reviews to develop Personal Development plan.
  • Be aware and comply with practice clinical policies and protocols.
  • Offer and provide, as appropriate to individual patient needs, all immunisations that are currently available as part of GMS.

Other Responsibilities

  • Working within Primary Care Networks: Heartwood Medical Practice is part of Swadlincote PCN and collaborative working is expected.
  • Awareness of and compliance with all relevant Practice policies / guidelines e.g. prescribing, confidentiality, data protection, health and safety.
  • A commitment to life-long learning and audit to ensure evidence based best practice.
  • Contributing to evaluation/audit and clinical standard setting with the practice.
  • Contributing to the summarising of patients records and read coding patient data.
  • Attending training and events organised by the practice or other agencies, where appropriate.

Qualifications/Requirements

  • Qualified GP
  • MRCGP
  • Vocational Training Certificate or equivalent JCPTGP
  • General Practitioner (Certificate of Completion of Training CCT)
  • Full GMC Registration
  • National Performers List registration
  • Appropriate defence indemnity (MPS/MDU)
  • Eligibility to practice in the UK independently

Experience

  • Experience of working in a primary care environment

  • General understanding of the GMS contract
