An exciting opportunity has arisen to join our warm, dynamic and supportive Stroke and Neurological Rehabilitation Team.

In this role, you will work as part of a multidisciplinary team that provides specialist assessment and rehabilitation for patients with a wide variety of acquired and progressive neurological conditions, including Stroke.

You will have a supervised caseload within the community, supporting provision of high quality, efficient and effective service to our patients, reviewing and evaluating rehabilitation programmes provided by therapists and supporting patient self-management. The role will also involve education and support of carers, social care and voluntary services involved in patient care.

You will be required to manage and prioritise their day-to-day work load and use your initiative. This will require good organisational, time management and communication skills and a flexible approach.

Our Stroke and Neurological Rehabilitation team members are very skilled and eager to support development of knowledge and skills within the service and to wider community teams. The service is really committed to professional development, joint working and development of leadership skills.

There are opportunities to develop specialist and detailed assessment and treatment skills within stroke and neurology, for supervision, in service training, leadership training and opportunities for service development.

To have excellent communication skills with patients, carers, relatives, colleagues and other services.

To be an effective team member respecting work of others and in representation of therapy profession.

For defined and delegated group of patients to undertake assessment of stroke and neurological patients with complex needs.

Participate and provide clinical treatments and rehabilitation programmes following professional assessment and guidance. This may relate to mobility, transfers, personal and daily activities of living, upper limb, cognitive, visual, perceptual, communication and sensory rehabilitation.

To maintain accurate, comprehensive and up to date documentation.
