Job Title: Dementia Care Manager

Reporting to: Deputy Manager

About the role:

Hallmark Care Homes’ dementia communities operate as a home within a home. As a Dementia Care Manager, you will lead and manage the team on that community, and be responsible for driving all aspects of the Hallmark dementia strategy, ensuring that your community meets the robust standards we have set for excellent relationship-centered dementia care.

With a focus on people, rather than ‘task’, it goes without saying that you must also have a passion for dementia care, and the confidence to challenge practices that create ill-being for the people in your care.

Supported by our Head of Relationship Centered Care, and Dementia Relationship Practitioner, and using your knowledge and experience of evidence-based dementia care, you will also act as the ‘go-to person for our residential and nursing communities within the home. You will develop links with local community groups involved in dementia care – in essence becoming the Hallmark ambassador for dementia in your local area.

You will be an excellent communicator, empathetic, and able to support family members and friends of people living with dementia to become partners in the care of their loved one.

Alongside this you will be responsible for ensuring that the dementia community meets all of the requirements of the CQC, and Hallmark’s quality and compliance framework, right through from assessment, care planning, quality assurance, health and safety, and recruitment and management of your team.
