Job Advert for two Nurses / Social Workers / other AHPs AfC Band 7 CHC Assessors

Permanent Contract

37.5 hours per week (job share and flexible working will be considered)

An exciting opportunity has arisen for two highly skilled Nurses / Social Workers or other AHPs to join the CHC / Complex Care team within NHS Bury CCG.

The post holders will lead on CHC caseloads; receiving referrals, liaising with GM and local teams and families, monitoring and reviewing patients to ensure patients are placed in the best cost-effective placements to meet their on-going health care needs.

The post holders will take a lead role in the identification of suitable care providers and will be responsible for ongoing review and reassessment of patients and commissioned packages of care. This will be with a view to ensuring that the package of care continues to meet assessed needs and that patients are maintaining as independent lives as possible and are safe and well.

The post holder will be expected to work with a wide range of NHS and non-NHS health providers, including General Practice, and with the Local, Authority across the borough.
