We are looking to recruit 3 new Clinical Support Workers to work in local GP Practices in the Gloucester, Cheltenham or Forest of Dean area (one post per area) and to be a part of a new service to support people who have a serious and enduring mental illness (SMI) in Gloucestershire. The individual needs to be motivated and have experience of working in Mental Health and/or Physical Health services and be able to deliver the annual physical health checks in local surgeries. Workload will be allocated from the SMI Register as the checks become due, the results will be documented and appropriate advice and onward referral to other services completed. In addition to providing the health checks, we want to ensure people have access to information and advice, interventions and/or signposting to appropriate local services. The post holder will work closely with GPs and nurses in the surgery and connect with the local community mental health services to deliver comprehensive annual physical health checks and co-develop care plans to enable service users to access the support they need to address their physical and mental health needs and promote recovery. The service will operate largely in GP surgeries, with some home visits where clinically indicated and the post holder will need to work automatously. Excellent communication skills and the ability to work flexibly to meet the service user’s physical health needs is essential.The qualification, training & experience requirements for the role are underlined in the Job Description/Person Specification.Closing date: 15th November 2022As a Health Care Assistant/ Support Worker you will be responsible for participating and assisting in the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. Health Care Assistant, Social Care Workers and other Healthcare Support Workers employed by the Trust are required to follow the guidance set out in Code of Conduct for Health Care Assistants/ Health Care Support Workers to provide safe and compassionate care of a high standard, and to challenge others who are not.You will receive direct supervision and partake in reflective practice / training on a regular basis and be required to meet the Care Certificate standards. All successful applicants will also have the opportunity to discuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post. This apprenticeship would include the Level 3 Diploma in Clinical Healthcare.This post requires the post holder to be able to undertake all statutory and mandatory training and post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.A valid UK driving licence plus the use of own transport for business purposes is essential.
