We are looking for forward thinking registered nurses who are passionate to support the changing face of community nursing. Are you confident in practice and able to support the frontline staff to ensure equity of work in the community setting?The purpose of the community nursing HUB is to triage and prioritise planned and unplanned visits. This may also involve signposting to the most appropriate wider services.As a registered nurse you will provide appropriate nursing advice to patients, family and carers’ in a community setting via telephone.You will work within your scope of practice and recognise your boundaries escalating queries to a District Nurse when required.You will be aware of the process for assessing, planning, implementing and evaluating patient care as delegated by the caseload holder.Be an effective member of the integrated team developing and maintain channels of communication within the team and with other agencies and professionals.This nursing element of the service will be available between 07.15 -21.15 seven days per week.This post is office based.
  • To take referrals from the nursing admin team where community nursing clinical expertise is required.
  • To reply to any staff queries that are within your scope of practice, specific patient support may require liaison with the caseload holder.
  • To triage and prioritise requests using clinical knowledge for any required visits which will involve allocation to staff. Any changes to staff work rota’s will need relaying to the staff member verbally. Any unplanned visits will always need to be allocated manually.
  • Work closely with the Malinko Coordinator.
  • To have/gain a good working knowledge of Malinko in order to support frontline staff and allocated visits.
Examples
  • For any patient requiring a visit from the Unscheduled Care Team (UCT), the referral should be passed to SPA in order for them to refer to the UCT via the SPA system. The referral does NOT need to telephoned through and can be handed over face to face in the SPA office.
  • For routine INRs and bloods should all be emailed to the generic inbox: nlg-tr.CTSRoutPhlebRequests (including any that come through to the DN HUB) for these to be picked up by phlebotomy team. The referral form should be completed where possible including NHS Number; name, address and request of what is required including date of blood test.
