Job Overview

The postholder will be directly accountable to the caseload holder.

The postholder will be responsible for assessing, planning, implementing and evaluating nursing care in the community.

The postholder will be expected to work collaboratively with the District nursing Team and other agencies to provide clinically effective care.

The successful applicant will need to have excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Conwy and Denbighshire working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment.

You must be able to travel between patients homes both within towns and rural areas in a timely manner.

This post is rotational over 24 hours.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
