The West London Community Forensic Team for People with Learning Disabilities & Autism (LDACFT) is looking for Band 6 community learning disabilities nurse to join the nursing team within the learning disabilities and autism team.

The post holder will be responsible for managing a caseload of people with learning disabilities and autism, with a wide range of health conditions.

As well as caseload management, the post holder will be involved in quality improvement projects to support the development of the nursing service, to improve the care provided to people with learning disabilities.

West London Trust and CNWL H&J work together to offer high quality specialist community forensic services across the 8 boroughs of the NW London CCG area (Ealing, Hammersmith and Fulham, Hounslow, Hillingdon, Harrow, Kensington and Chelsea and Westminster) through the SCFT model.

The Learning Disability and Forensic Community Service, will extend within the SCFT model across all 8 boroughs in a more systematic way for the provision of LD&A community forensic services. This ensures that LD&A community forensic services provide a consistent and joined up service across the NWL ICS.

MAIN DUTIES AND RESPONSIBILITIES

Clinical:
  • To manage a caseload of people with a learning disability with a variety of health conditions.
  • To work closely with other health services to ensure people with a learning disability have access to mainstream health services.
  • To ensure reasonable adjustments are made for people with learning disabilities, to access health and social care services.
  • To lead on service development projects to improve health outcomes for people with learning disabilities accessing the service
  • To work as part of the multi-disciplinary team with professionals from other disciplines
  • To make referrals to other health and social care disciplines as required
  • To independently undertake initial nursing assessments of the health needs of people with learning disabilities
  • To plan, implement and evaluate the nursing needs of people with learning disabilities
  • To assess risk and implement risk management plans
  • To raise safeguarding concerns as they arise
  • To be involved in the completion of mental capacity assessments
  • To provide training to people with learning disabilities, care providers and healthcare professionals relevant to individuals health needs
  • To create resources to provide to people with learning disabilities with accessible, easy read information relating to their health needs
  • To support service users with a wide range of health needs such as, epilepsy, diabetes, cancer, dementia etc.
  • To support local GP’s to undertake annual health checks of people with learning disabilities as required
  • To support the implementation of nursing and MDT pathways in to normal nursing practice
  • To support in the provision of desensitisation programmes to support blood tests, and Covid 19 vaccinations
  • To undertake LeDeR reviews for those who have passed away with a learning disability
  • To act as health duty worker on a rotational basis to support hospital admission and discharges and management of health concerns in the community
  • To joint work with the local hospital services and acute liaison nurses to support hospital avoidance planning, hospital admissions and hospital discharges
Management:
  • To provide clinical supervision to junior members of staff as required
  • To support student nurses and apprentice nursing associates during their practice placements
  • To attend regular supervision with your line manager
  • Utilise time management skills to effectively manage a caseload of people with a learning disability
Training and Education:
  • To engage in continuing professional development, which will be discussed and agreed with manager
  • Attend and keep up to date with all statutory and mandatory training as required by the trust
  • To ensure knowledge and skills are upheld in line with Nursing Midwifery Council standards and requirements
  • To complete nursing revalidation every 3 years
Communication:
  • To confidently and effectively communicate with people with a learning disability
  • To adapt communication style according to audience - to communicate with family members, carers and a range of health care professionals.
  • To confidently communicate in a variety of settings, including in multidisciplinary team meetings, homes, hospitals and care homes to advocate for service users with a learning disability
  • To use reflective practice within the workplace to develop skills
  • To confidently use IT skills - to write comprehensive nursing reports, letters and emails to a variety of different audiences, as well as then adapting these reports to accessible versions
  • To manage electronic diary
  • To upload case notes in a timely manner
Information Management:
  • To be aware of and adhere to all local trust policies
