Main duties of the job

There will be a requirement to provide a high level of clinical expertise, knowledge and skills, taking a lead role with advanced clinical assessment, diagnosis, treatment and management of patients who are experiencing an acute episode of their chronic condition or are an elderly medically frail deteriorating patient in the community and or a high intensity user of services. Included in the role will be in reach support to community hospitals with GP led beds, support to care homes and to work closely with Primary and Secondary Care. The focus will be to support patients to remain safely in the comfort of their own homes for longer, prevent hospital admissions, facilitate early safe discharge and reduce GP contacts.

The post-holder will be an autonomous practitioner and proactive in pioneering and developing this role further in response to challenging healthcare needs across the community establishing close working relationships with Primary and Secondary Care. The role necessitates a high level of accountability ,clinical leadership, education, and evidence based practice. The successful applicant will need to be diligent, flexible in approach, organised, have excellent interpersonal skills with an aptitude for service improvement.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
