Community nurses provide an essential role to deliver care to patients in their own homes, avoiding hospital admission and allowing people to stay in their homes longer.We are seeking a motivated, dynamic and passionate, Senior Community Nurse for our Urgent Care Response team. Based with our Single Point of Access (SPOA) Service at Queensborough House in Dunstable, you will triaging and assessing urgent calls that come into the service and allocating the appropriate response to these calls. This role is predominantly an office based triaging role.You will need to be a strong team player as well as an autonomous practitioner. Focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, and develop individuals to deliver consistently high quality services.You must have a driving license and have access to a vehicle in order to carry out your duties.
  • Provide high quality complex clinical triage using knowledge, skills, critical thinking and clinical assessment utilising criteria and clinical pathways.
  • Provide healthcare advice supported by clinical pathways and criteria, ensuring timely onward referral to the appropriate Primary Care at Home team
  • Be available to staff a 7 day a week service model
