The Urgent Care Hub is expanding and we are looking for a Band 6 Clinical Lead to join the team (full time and part time opportunities available) managing the functional and organic adult mental health beds across Somerset. The Urgent Care Hub currently consists of a Team Manager and 2 Clinical Assistants and operates 9-5 Monday to Friday but these hours are likely to extend on successful recruitment to this post.

The Urgent Care Hub role is to support admissions and discharges from the adult and older persons’ wards and provide a clear pathway throughout the patient journey with a clear focus on flow.

The hub receives all requests for admission to hospital from HTT, IDSS and AMHPs, discusses the role and expectations of the admission before identifying the most appropriate bed for the patient. The team is in daily liaison with all of the wards, home treatment teams and other agencies including the voluntary sector, to ensure that potential barriers or delays to discharge are kept to a minimum. The aim is to ensure that patients will have a reduced length of stay leading to lower bed occupancy levels and ensuring that patients receive ongoing care within the community setting. The hub also manage the admissions into our "Step Up" and "Step Down" beds in Yeovil and Wells respectively.

Team members must have excellent interpersonal, communication and organisational skills. They must be flexible, creative and responsive to managing the changing needs of the service which will be challenging at times. A "can do" attitude, the ability to build relationships with other services, and an aptitude for making sometimes difficult decisions is required. It is necessary that the applicant is mobile across the county and therefore must have a driving license.
