The Individualised Care Team are seeking an experienced Continuing Healthcare Assessor looking for the next step in your career.

This is an excellent opportunity for a suitably qualified Registered Nurse to join our busy Continuing Healthcare Team.

You will have significant knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (Revised 2012), and its application. Ideally, you will also have knowledge and awareness of social work systems and frameworks.

As part of the CHC Team, you will lead and manage the work of the CHC Nurse Assessors workers to deliver CHC assessments in line with the National Frameworks for NHS Continuing Healthcare and NHS Funded Care.

You will be able to manage your time and the time of others in the team. You will be able to work flexibly in a pressurised environment.

You will ideally have experience of managing staff and of managing change. You will have relevant experience in complex case management and, drawing on previous skills and knowledge, and be confident about what is required to deliver high quality care to patients.

You will need to demonstrate a high level of interpersonal and communication skills, including the application of tact and diplomacy in challenging expectations and complaints, to liaise effectively with colleagues, health and social care professionals, patients’ family members and others.
