Are you a Registered Nurse. Would you like the opportunity to support individuals with their ongoing health needs. BSW CCG are transforming their CHC delivery service and are seeking a CHC Operational Lead for the Swindon Locality.
  • The National Framework for Fully Funded NHS Continuing Healthcare and NHS Funded Nursing Care (revised October 2018)
  • Personal Health Budgets.
  • CHC Operating Plan and Quality Assurance Framework
The successful applicant will be responsible for ensuring that the needs of those assessed to meet the eligibility for CHC are being met appropriately and proportionally by high quality commissioned care. This will include managing a team of CHC Clinical Leads and Case Coordinators.
The successful applicant is required to have a thorough understanding of The National Framework for Fully Funded NHS Continuing Healthcare and NHS Funded Nursing Care (revised October 2018) As a senior manager for the service, you are responsible for supporting the Head of Operations and Clinical Quality for CHC/FNC in ensuring that the process for establishing and ratifying eligibility is in line with statutory requirements and national guidance is compliant, transparent, and robust. You will take daily responsibility for the operational delivery of CHC/FNC working closely with social care colleagues to maintain the established integrated approach to assessments. You will take part in the joint delivery of CHC training, so a sound knowledge base of the National Framework and local processes is essential. There will be an expectation for the successful applicant to represent BSW CCG at Independent Review Panels held by NHSEI both presenting cases on behalf of BSW CCG and sitting as an independent representative for other CCG cases.
