Main duties of the job

An exciting opportunity has arisen for suitably qualified and enthusiastic nurses to join this well established cardiac rehabilitation service. The post holders will join the multidisciplinary team to contribute to the delivery of cardiac rehabilitation care in a variety of settings. The role will involve leading on and implementing cardiac rehabilitation interventions in Gwynedd and Anglesey with requirements to travel across Gwynedd and Anglesey.

The client caseload includes those following acute coronary events and /or coronary intervention and those with heart failure. The postholders will promote independence, confidence and improved quality of life for both the client and their family where necessary. You will require excellent communication and motivational skills and be able to work effectively both within a team and on your own.

This posts will be suitable for those with relevant experience in cardiac nursing and/or nursing individuals with long-term conditions in primary or secondary care. It may attract nurses from a variety of settings eg coronary care, emergency department, practice nursing, district nursing.
