The opportunity has arisen to a Dietetic Support Worker to work under the direction of the Highly Specialist Public Health Dietitian. They will deliver a brief interventions to patients at high risk of developing Diabetes.

Post holders will also deliver a range of community based initiatives; utilising a range of agreed approaches to support positive changes to food, nutrition, and physical activity behaviours. They will support the local delivery and evaluation of Nutrition Skills for LifeTM services.

The successful candidates will receive a range of training opportunities and work alongside fellow Dietetic Assistant Practitioners in Public Health Dietetics, under the guidance of the Senior Public Health Dietitians. They will receive weekly supervision sessions with a registered dietitian and will work closely with a range of partner agencies to promote positive healthy lifestyle behaviours.

All post holders will work to an agreed framework with a degree of autonomy and will be responsible and accountable for tasks delegated to them. This will involve having their own caseload and working productively with a range of partners and organisations in a complex community environment.

The bank worker will work as a member of the Diabetes Prevention Programme Team under the direction of the Highly Specialist Public Health Dietitian . The range of responsibilities will be in support of and supervised by a Registered Dietitian (RD) and will involve working within a variety of Community and Primary Care settings across the Cwm Taf Morgannwg Health Community (as appropriate to the needs of the service).

The bank worker will support the delivery of an integrated care and support programme for people at high risk of developing type 2 diabetes -

The bank worker will conduct type 2 diabetes risk assessment health checks, supported with a customised computer software programme, on residents invited to the programme from their GP practices.

The bank worker will be expected to undertake appropriate training to use the software and also to develop excellent communication and behaviour change skills which will enable them to support and motivate patients to make positive lifestyle changes, identifying their own diabetes risk factors and enable them to reduce and manage their risk
