Leicestershire Partnership NHS Trust (LPT) provides a range of community health, mental health and learning disability services for people of all ages. Delivered through over 100 settings from inpatient wards to out in the community, our 6,500 staff serves over 1m people living in Leicester, Leicestershire and Rutland.
Leicestershire Partnership NHS Trust is working with Adult Social Care across Leicester, Leicestershire and Rutland (LLR) to deliver a newly commissioned Home First service offer alongside core Community Therapy.
We are looking for an enthusiastic Band 6 Physiotherapists to be part of the integrated health and social care team in Rutland. You will be embedded within the Rutland reablment team and will deliver high quality care to the residents of Rutland.
As a result of recent service changes there has been investment in therapy staffing, ensuring that services can be delivered by the right therapist, in the right place at the right time, and to ensure that the local population across LLR receive timely high quality therapy services. We have also recently been successfully announced in LLR as an accelerator site for the Ageing Well Programme - this is a really exciting time to be joining this newly formed service bringing together an integrated health and social care offer delivered by community-based nursing, therapy and adult social care teams.
This post will allow you to use your clinical physiotherapy skills to:
deliver high quality reablement, rehabilitation and recovery to people within their own homes within the community. provide urgent care to patients in crisis to help avoid unnecessary hospital admissions help people get home from hospital quickly and provide them with rehabilitation and reablement to help restore their health, wellbeing and independence

The successful candidate will join our senior therapy team to provide high quality clinical physiotherapy interventions, advice and management and to support the senior leadership team in developing core and Home First therapy services across Community Health Services.
Applicants should have a broad range of knowledge and skills gained through prior band 5/6 experience and must be able to demonstrate their skills in supporting the clinical development and training of an established team of junior therapists and technical instructors in line with evidence based practice.
As a senior clinician you will be expected to maintain an independent caseload of patients as well as contributing to teaching and supervision of more junior colleagues.
The successful post holder will undertake day to day management of the a therapy caseload, and you will be encouraged to participate in service development, with regular supervision, teaching and peer support in line with your personal development plan.

We are looking for a determined and self-motivated individual who can make a difference to the care of patients from all backgrounds, and to promote therapy services within the trust.
