Main duties of the job

As an Advanced Nurse Practitioner you will be working as a member of the Urgent Primary Care team to provide care to patients referred from Primary Care, ED, Minor Injuries, Out of Hours and Community Pharmacies with acute onset, single issue, urgent care needs. You will assess patients’ clinical needs, providing safe and effective patient care delivered utilising clinical skills, incorporating history-taking, physical assessment, diagnosis and liaising where appropriate with other healthcare professionals. Central to the role will be the use of critical thinking and advanced decision making skills in order to accurately assess and manage patients with a wide range of clinical conditions/illnesses, this may be over the phone (or via other forms such as video consultation) or face to face in the Centres. We are seeking nurses with a proven record/special interest in caring for patients with minor illness and some experience of dealing of Chronic Disease Management.

Ideally, the candidate will hold an MSc in Advanced Clinical Practice, but are willing to consider applications from individuals who are currently undertaking, and will complete, the MSc Advanced Clinical Practice qualification whilst in post.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
