As a 5-19 Screener, you will be responsible for the scheduling of the NCMP, vision and hearing screening in your locality and to ensure the service is delivered to the school age children in Norfolk. You will be working as part of a team to deliver the service and this will be in a school setting or home setting. You will effectively communicate with schools to support the delivery of the programme. The role would require accurate recording of childrens results and process of the data collected in school.

You will liaise with parent/carers on a regular basis to inform them of their childs results and if further testing needed or an onward referral. A full training programme will be provided to support the successful applicant. Job Purpose: To work as part of the 5-19 Healthy Child Pathway team, with a key role in supporting the delivery of the National Child Measurement Programme (NCMP) working under the guidance of the national programme and maintaining good liaison with schools. To work under the guidance of the 5-19 team in delivering the Healthy Child Programme (HCP) to children of school age in schools, clinics and other settings to promote health of children and prevention of ill health.

This will include the provision of a screening service for hearing and vision on school entry and ad hoc screening as required. Main Duties and Responsibilities: Be responsible for organising provision of hearing and vision screening and monitoring of height and weight for children of school age within the Norfolk area under the direction of the Healthy Lifestyles/ NCMP Screening Co-ordinator. Undertake routine screening for hearing and vision within schools for children aged approximately 5 years, according to the Trust Clinical Standards and participate in the training and subsequent observations required. Appropriately refer children to the Specialist Community Health Practitioner or other registered nurse within the 5-19 team, in partnership with parents, where a problem has been identified, according to the protocol.

Inform parents of the results of screening tests within an agreed time frame, and liaise about any further testing or referral. Undertake routine weight and height measurements of children in year R and 6 in line with the National Childhood Measurement Programme. Prioritise referrals for audiograms according to levels of need/urgency. Undertake Pure Tone Audiograms for children referred into the service, according to agreed standards and guidelines.

Undertake height and weight assessments for individual children, as referred by the 5-19 team. Report and record any concerns regarding safeguarding children to the Healthy Lifestyles/ NCMP Screening Co-ordinator, Healthy Weight Pathway Lead, Team Leader/Safeguarding Lead following the safeguarding concern pathway for screeners the same working day. Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice.

Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport. This is subject to the provisions of the Equality Act 2010, and therefore if the postholder requires a reasonable adjustment due to a disability then this will be considered. People Management: Maintaining own professional development and requirement to take part in appraisal and management/clinical and safeguarding supervisions.

Clinical and Practice Governance: Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of contact must be undertaken with regard to the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes. The post holder must adhere to infection control policies and procedures.

It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust. The post holder must participate in clinical and other audits as required. Provide patients and their families /carers with information on standards they should expect from the team.

Participate in clinical supervision and safeguarding supervision on a regular basis. The post holder is required to participate in relevant emergency preparedness process for their team.
